Instructions for Completing the Claim Form
1. Eligibility
- To be eligible to receive a settlement benefit you must have purchased one or more “Covered Products” from Bed Bath & Beyond between August 1, 2000 and November 9, 2007 (the “Class Period”). To verify that the item you purchased is a Covered Product, you should consult the Covered Product List. You may also search the Covered Product List by using the UPC Code from either your receipt, packing list or from your product packaging; or by using the product description from you E-Mail shipping confirmation. For help in locating the UPC Code on your proof of purchase, click on the Proof of Purchase links below. For help in locating the product description on your email shipping confirmation, click on the E-Mail Shipping Confirmation link below. 2. When to file a Claim
- For each purchase of a Covered Product you made during the Class Period, you may file a claim for one of three different settlement benefits, depending on the purchase documentation you have retained and whether or not you wish to keep the Covered Products that you purchased.
- File one Claim Form for each store receipt, e-mail shipping confirmation, or packing list evidencing your purchase of a Covered Product during the Class Period. If you do not have any of these items, but you have a Credit Card or other Bank Statement showing a purchase of more than $125.00 from Bed Bath & Beyond during the Class Period, file one Claim Form for each Credit Card or other Bank Statement. If you do not have any form of proof of purchase, file one Claim Form for each date you purchased Covered Products during the Class Period.
- If multiple Covered Products are included on a Claim Form, you may only choose one type of settlement benefit for those products (e.g. if you have 2 Covered Products on a receipt and include both products on your Claim Form, you may request 2 refunds or 2 $10 gift cards, but you cannot request one refund and one gift card).
3. Types of Settlement Benefits
- A Refund of the purchase price for each Covered Product purchased; or
- A $10.00 Gift Card from Bed Bath & Beyond for each Covered Product purchased; or
- A 20% Discount Certificate off any Bed Bath & Beyond purchase up to $250.00 (maximum discount of $50.00) for each Covered Product purchased.
4. Instructions
- Section I: Claimant Information
- Enter any corrections to your name or address in the spaces provided. If you change your address after you have filed your claim, please provide the Settlement Administrator with your updated address.
- Section II: Enter Your Claim Purchases
- Determine which settlement benefit you are choosing and what type of documentation, if any, you are providing with your Claim Form. You must circle your Covered Products on your enclosed Proof of Purchase or your Sales Transaction on your enclosed Credit Card or other Bank Statement. - Enter your Purchase Date from your documentation or if you do not have any documentation, from your best estimate.
- Enter the City or Town and State where you made your purchase next to Store Location. If you made your purchase from the Bed Bath & Beyond website, please enter "Web".
- Enter the Description of the Covered Product, as it appears on your Proof of Purchase. If you do not have a Proof of Purchase, enter the description as it appears on the Covered Products List, available here.
- You must also provide the Quantity, Unit Price and Total Purchase Price for each Covered Product for which you are making a claim.
- If there are not enough lines, continue your list on a separate sheet of paper. Print your name and Claim Number on the top of the additional sheet.
- Section III: Certification
- You must sign and date your completed Claim Form under penalty of perjury. Failure to do so may result in the denial of your claim. All information entered in support of your claims is subject to investigation, verification and challenge by Bed Bath & Beyond and the Settlement Administrator.
- Mail your Claim Form with the required documentation with a postmark no later than May 13, 2008 to BBB Thread Count Settlement Administrator, P.O. Box 6159, Novato, CA 94948-6159.
